Managing PPP Forgiveness

Effective immediately, each Borrower will be required to submit the Forgiveness Borrower Acknowledgement (link below) in addition to the necessary SBA documents as part of the forgiveness process.

As a reminder, when you are ready to apply for forgiveness, please send the following documents to PPP@myaffinitybank.com:

- 1 – Forgiveness Application – one of the following:
  - 3508S (if your loan amount is $150,000 or less)
  - 3508EZ (use the instructions to see if you qualify if your loan amount is greater than $150,000)
  - 3508 (use if you do NOT qualify for From 3508S or Form 3508EZ)

- 2 – Completed Forgiveness Borrower Acknowledgement Form
