Managing PPP Forgiveness

Effective immediately, each Borrower will be required to submit the Forgiveness Borrower Acknowledgement (link below) in addition to the necessary SBA documents as part of the forgiveness process. As a reminder, when you are ready to apply for forgiveness, please send the following documents to PPP@myaffinitybank.com:
    • Forgiveness Application (choice of Form 3508, Form 3508EZ, or Form 3508S)
    • Proof of payment for any expense you are requesting forgiven (see instructions for what is acceptable)
    • Proof of obligation for non-payroll expenses you are requesting forgiven (see instructions for what is acceptable)
    • If using Form 3508 or Forms 3508EZ, proof of payment for any expense you are requesting forgiven (see instructions for what is acceptable)
    • If using Form 3508 or Forms 3508EZ, proof of obligation for non-payroll expenses you are requesting forgiven (see instructions for what is acceptable)
    • Completed Forgiveness Borrower Acknowledgement (link below)
