Forgiveness

Effective immediately, each Borrower will be required to submit the Forgiveness Borrower Acknowledgement (link below) in addition to the necessary SBA documents as part of the forgiveness process.

As a reminder, when you are ready to apply for forgiveness, please send the following documents to PPP@myaffinitybank.com:

• • 1 – Forgiveness Application – one of the following:
    • 3508S (if your loan amount is $150,000 or less)
    • 3508EZ (use the instructions to see if you qualify if your loan amount is greater than $150,000)
    • 3508 (use if you do NOT qualify for From 3508S or Form 3508EZ)

• • 2 – Completed Forgiveness Borrower Acknowledgement Form
